Buckeye PACE - Ashtabula

PACE (Programs of All-Inclusive Care for the Elderly) is an all-inclusive, home- and community-managed care model for individuals aged 55 and older. That means PACE takes care of everything the person needs, not just doctors and medications. Services are provided in the person’s home, at a local PACE Center, or in the community. PACE brings all aspects of care together in one place, so caretakers can focus on the moments that matter most.
