Forms of identification required to apply for a Library Card in person, or to pick up a Library Card after an electronic WebReg application.

We require verification of identity and present address for adults wishing to ...
- register for a library card for them or their child.
- renew an expired library card.
- or replace a lost, stolen, damaged, or destroyed library card.
Suggested forms of photo identification that state the person's name and address include, but are not limited to ...
- Ohio or Out-of-State Driver's License.
- ID Card from school, work, or armed forces.
- Passport.
Although a photo identification is preferred, we will accept a combination from each of the following categories:

Name Verification.	Address Verification.
Any other non-photo ID card with the patron's name and address. Auto registration/title or car insurance ID. Birth Certificate. Checkbook / current bank statement. Current payroll stub. Unexpired major credit card. Voter registration card.	Checkbook. Current tax receipt. Utility bill (dated within one month). Lease agreement with address (dated within one month). Major credit card bill (dated within one month). Welfare / medical assistance card.
We cannot accept the following forms of "identification." Personal mail, Social security card, Phone book listing, City/county directory listing.
